Panelist and Moderator Forms

We are making use of three speaker forms as a means of streamlining the process. We need different information from our speakers at different points during the event organization process and we have found using a single form creates a bottleneck and delays in obtaining the necessary information. Form 1: APFM 2025 Conference Speaker Agreement

We look forward to having you present at the 2025 APFM Conference! Please complete the below form at your earliest convenience. This form is your confirmation to your role in the conference, as well as time and date of your presentation, as summarized in the email you received from our conference committee members.

Would you prefer a Zoom webinar or meeting?

Zoom Webinar
Only you and other panelists will appear on the screen. You will not be able to see participants or make your presentation interactive. Participants will be able to ask questions via Q&A function though. Best for panels and non-interactive presentations.

Zoom Meeting
Speaker and attendees will be able to be seen on screen. Will be able to interact with participants and include them in the conversation. Best for interactive presentations.

Please provide any presentation/workshop materials you would like to share with Conference attendees. These will be made available on the APFM Conference website for attendees before the Conference. We prefer all moderators collect materials from their panelists and submit at one time if possible.
